Baby – First Ever Registration

How to Register

1. Check that you live within our practice boundary. We will not register people living outside this area unless you have been allocated to us by the Healthboard.

2. Look out your Baby’s Birth Notification Card that you will have received from Hospital or the NHS Registration Form (EC58) that you will have received when you registered the birth. This can be uploaded in the registration form below or brought into the practice to be checked

3. Complete the Registration form below. You should then be registered within 48 hours.

If you don’t have a mobile number, please state ‘none’. However, we cannot register you without a mobile or landline number
