How to Register
1. Medication. Ensure you have at least 1 month’s worth. It can take up to 4 weeks for your medical notes to be transferred to us making it difficult for us to issue you repeat medication immediately. Please ensure you have at least 1 month of medication from your previous practice before registering with us.
2. Services. Ensure you are happy with the services that we offer. All GP practices offer general medical services (GMS) but practices differ in their Shared Care Agreements (SCAs) with hospital clinics. If you take high risk specialist medication (eg for ADHD, organ transplants or gender affirming hormones) and you register with us, we may need to refer you to a specialist hospital clinic for ongoing prescriptions.
3. Practice Boundary. Check that you live within this. We will not register people living outside this area unless you have been allocated to us by the Healthboard.
4. Identification. This is no longer needed to register with a practice. However, if deemed necessary, we are able to ask for identification at any point to confirm your identity and your address for reasons of data protection and practice boundaries.
5. Complete the Registration form below. We will notify you once you are registered.
