Protecting Personal Health Information

For the purpose of your present and future treatment, your personal and care details will be documented. This may be on paper (your medical records) or on computer.

At all times great care is taken to ensure that high standards of confidentiality are maintained in respect of records held.

We use the information which we hold on patients for their benefit, e.g. reviewing and monitoring patients with heart disease.

We may use this information for other reasons, such as research, audit, appraisal, payment verification, training and planning purposes.

Strict confidentiality is maintained when records are used for these purposes, but you are entitled to object to such use of your records and this objection will be respected.

Please contact the Practice Manager if you have an objection.

There is a leaflet “Protecting Personal Health Information” on display in the waiting room or available at Reception.
