Chronic Disease Management for: – Type 1 Diabetes (managed by hospital clinic) – Hyperthyroidism (managed by hospital clinic)
Antenatal Clinics with the Midwife Child Health and Development Baby Clinics (6 week checks) Cervical Smears Family Planning services including implants and coils Joint Injections Men’s Health Women’s Health Palliative Care Shared care for selected specialist medications
Home Nursing Services (Provided by the District Nurses)
Non-NHS Examinations. Examinations for special purposes (e.g. HGV or insurance) are not directed related to your health and therefore not funded by the NHS. The practice will carry out these services however there is a charge.
Reports and forms. Except Sick Notes, Medical Incapacity Forms (AWI) and Department of Work and Pension (DWP) forms, all other reports and forms are not funded by the NHS and will therefore incur a charge.
