How to Register
1. Fill out the registration forms. Please complete these below and then print out and sign. Alternatively, you may request a paper copy from reception for completion.
2. Identification. Look out two forms of identification.
The first must contain a photograph (e.g. passport, driving licence or ID card)
The second must contain your current address (e.g. utility bill showing your name, rent book, council tax notification, TV Licence, etc).
For newborn children, the parent or guardian ID is sufficient.
3. Attend with the forms and ID. Bring the completed registration forms and your two forms of identification to Reception between 10 am – 4 pm (staff are prioritised for immediate care at other times).
4. New patient appointment. All new patients over the age of 16 who are on repeat medication are required to attend a new patient medical appointment with the Practice Nurse. Please arrange this appointment with reception.
Non-urgent advice: Please Remember
You are not fully registered with the practice until you have completed and signed the New Patient Registration Form, produced the required identification and attended a New Patient Medical appointment with the Practice Nurse (if required).
